Application Information

FOR THE 2008-2009 SCHOOL YEAR FOR CHILDREN WHO WILL BE 3 ON OR BEFORE SEPTEMBER 1, 2008

NO WAITING IN LINE! 

Bring address verification and come to Ballenger (1110 40^th St.) June 2-9^th between 8:00 a.m and 3:00 p.m. Parent/Guardian will fill out a short application form. On June 10^th we’ll randomly draw those application forms and call parents of selected children to pick up an application packet. When picking up a packet an appointment will be made for you to return on July 23^rd with all the REQUIRED DOCUMENTATION (see qualifications Qualifications.htm) to see if they qualify for the program. Children who don’t receive an application packet will be placed on a waiting list in the order drawn. ONLY the parent or legal guardian may fill out the forms.

CHILDREN WHO WILL BE 4 ON OR BEFORE SEPTEMBER 1, 2008

Pick up an application packet at Ballenger (1110 40^th St.) between 8:00 a.m. and 3:00 p.m. beginning June 9^th throughout the summer. Parent / Guardian will pick up the application packet and make an appointment to come back in July with all the REQUIRED DOCUMENTATION to complete their paperwork for the program. Only the parent or legal guardian may fill out the forms.
