Life Insurance Coverages

Lafayette Life Insurance - Term Life and AD&D | Northwestern Term Life (Optional) | Cincinnati Life (Optional Permanent Life)

Lafayette Life Insurance - Term Life and AD&D

Through the Lubbock Independent School District employee benefits package you have $5,000 basic term life and $5,000 AD&D coverage. Employees on the Hospital Income Plan (HIP) have $25,000 term life and AD&D insurance coverage. This coverage becomes effective on the 31st day of employment if your application is on file in the Risk Management Office.

Optional Life Insurance and AD&D - Lafayette

New employees may purchase Lafayette Life Insurance with no evidence of insurability during first 31 days of employment. After this period of time, evidence of insurability will be required. This Optional Life coverage may be purchased in increments of one, two, three or four times the employee's annual earnings not to exceed a maximum amount of $100,000. Employees may cover spouses up to $50,000, not to exceed 50% of the employee's amount. Dependent children may have $5,000 or $10,000 coverage, if employee has coverage. The full amount of your Optional AD&D coverage is equal to the amount of Optional Life coverage purchased. Premiums for this optional coverage will be made through payroll deduction.

Conversion Privilege

If your life insurance terminates because you are no longer eligible under the provision of the plan, you may, within 31 days of coverage termination, convert your former coverage to an individual policy without further evidence of insurability.

This represents a brief outline of the coverage. Please refer to the LISD Employee Group Health and Group Life Plan booklet for benefit provisions and limitations.

Northwestern Term Life (Optional)

You may purchase from $5,000 to $100,000 coverage for you, your spouse or you and your spouse. The policy is yearly renewable term life insurance to age 70. The plan includes waiver of premium due to disability, double indemnity in the event of accidental death prior to age 65 and conversion to a permanent plan of life insurance at any time prior to age 70.

Cincinnati Life (Optional Permanent Life)

This provides an opportunity for you to purchase through payroll deduction either Universal Life or Whole Life Insurance for yourself, spouse, children or grandchildren. This insurance provides an accumulating cash value as well as death benefits. If you leave employment with LISD this coverage can be converted to an individual policy at the same rates you are paying in the group.
