Continental Casualty AD&D
This coverage is available to all active, full-time employees (scheduled to work at least 30 hours per week) age 18 through 69.
This insurance covers you 24 hours per day, on or off the job. You may select benefits from $10,000 to $250,000 in multiples of $10,000. Benefits are payable in addition to any other insurance that may be in effect at the time of an accident. Face amount of the policy is for loss of life due to an accident. Specified amounts, up to the full face amount of the policy, are for dismemberment, loss of sight, speech, hearing, etc.
Family Coverage
Your spouse age 18 through 69 and all unmarried, dependent children age one through 19 (or 23 if attending school full-time) can be insured for a portion of your benefit amount for a small additional monthly cost.
- Spouse - 50% if no eligible children
- Spouse - 40% if eligible children
- Children - 15% each if no eligible spouse
- Children - 10% each if eligible spouse is covered
The effective date of coverage will be the first of the month after your enrollment form is received. Coverage will continue as long as you remain an LISD employee, pay your premiums and have not reached age 70. Premium payments will be deducted through the payroll deduction plan.

AFLAC Cancer and Intensive Care Plan
An optional cancer, intensive care and dread disease insurance policy is available for employees and families through our payroll deduction plan. Family coverage includes insured, spouse, and dependent, unmarried children to age 25. This plan pays benefits in addition to any other hospitalization or medical insurance coverage effective at the time a claim is submitted. The coverage becomes effective 30 days following the approval of coverage by the company.
The cancer plan provides:
- $2,000 - First occurrence benefit (Increases by $500 each year your policy is in force).
- $300 - Per day for hospital confinement for cancer treatment.
- Up to $275 - Per day for outpatient treatment or hospital confinement, chemotherapy and radiation.
- Up to $5,000 - For cancer surgery
- Wellness Benefit
- Bone marrow transplantation
- Several plans are available through this company for cancer coverage. They vary according to the amount of coverage desired and the amount of premium one wants to contribute. These additional plans are available through the LISD Risk Management Office.
Dread DiseaseRider
The optional dread disease rider provides the following when you or insured family members are hospitalized for treatment for specified diseases:
- $200 - per day
- $100,000 - Lifetime maximum
- Examples of covered diseases (see plan in the LISD Risk Management Office for a complete list)
- Cystic Fibrosis
- Encephalitis
- Muscular Dystrophy
- Sickle-Cell Anemia
- Tuberculosis
- Others listed on brochure
- Benefits are paid directly to you, unless you assign them.
Hospital IntensiveCare Option
Hospital Intensive Care Option is available along with the basic coverage plan and provides the following benefits:
- Day 1 through 7- $600 per day, Days 8 through 15 - $1,000 per day when confined in Hospital Intensive Care, Coronary Intensive Care, or Neonatal Intensive Care Units, and other benefits available as listed in brochure.
- Benefits will be paid for a maximum of 15 days for each period of ICU confinement. There will be a 30 day separation period before benefits would again be available for the same illness.
- Benefits are paid regardless of other insurance.
- Benefits are paid directly to you, unless you assign them to a hospital or physician.

Washington National Group Disability
New LISD employees are eligible to enroll provided application is made during the 60 days following the date of employment.
For all Plans, Proof of Good Health is required, but such Proof of Good Health is waived up to a specified benefit level during the installation period and during declared open enrollment periods if the necessary percentage of enrollment is reached. Proof of Good Health is also waived for new employees who are eligible and who make timely application. See booklet for levels of coverage that always require Proof of Good Health satisfactory to the company.
Eligibility
An open enrollment period is scheduled every three years for employees who wish to enroll without the requirement of evidence of insurability.
Washington National Base Plan-Plan 1 (Plan 1 is required in order to select Plan 2, Plan 3, or Plan 4).
Accident Benefit
The monthly benefit of the level selected (see schedule) is payable during each period of total disability. Benefits start on the first full day of disability and continue to age 65. If accident, disability commences after your 65th birthday, benefits will be payable for a maximum of 60 months, or to age 70, whichever is earlier.
Sickness Benefit
The monthly benefit of the level selected (see schedule) is payable during each period of total disability. Benefits start on the fourth day or thirty-first day of disability depending on waiting period you select and continue for a period of time not to exceed 36 months for each disability. Disability caused by pregnancy will be payable the same as any illness.
Integration withOther Income Sources
After the first 24 months of accident and sickness payments under this plan, the monthly benefit under this plan will be reduced by OTHER INCOME SOURCES (listed below).
- any Group Disability Insurance Plan
- Social Security (including Primary and Dependents' benefits)
- State Teachers' Retirement or Disability Plan
- Workers' Compensation, or any similar laws
- any other program providing disability or retirement benefits
The Company will offset against any of these benefits whether or not such benefits are applied for. However, the benefits under this plan will not be reduced to less than 25% of the amount of the benefit otherwise payable.
Lower monthly rates are available if an insured wishes to have coverage for 1st day of accident and 31st day of illness instead of 1st day of accident and 4th day of illness. (See booklet for rate structure).
Non Disabling Injury Benefit
A benefit will be paid in the amount of the actual charge made by the doctor for medical treatment required and received within 48 hours for any accident that does not cause disability, but not to exceed $100 for any one accident.
AD&DBenefits
The Accidental Death and Dismemberment Benefit is payable in the event of loss through accidental bodily injury of both hands, both feet, sight of both eyes, loss of any two such members or of life.
One-half the Accidental Death and Dismemberment Benefit is payable for loss of a hand, a foot, or the sight of an eye.
Double the Accidental Death and Dismemberment Benefit is payable if the loss occurs while a passenger in or on public transportation (including the platform or steps).
The Accidental Death and Dismemberment Benefit increases 10% of the original amount each year for ten years provided your coverage is continuously in force and the premium is paid when due.
The Accidental Death and Dismemberment Benefit is paid in addition to any other insurance that may be payable.
The Accidental Death and Dismemberment Benefit is payable only for loss that occurs within 90 days of the date of the accident unless state statutes require otherwise. Losses due to suicide or attempted suicide and losses due to self-inflicted, intentional injuries are not covered. If more than one specified loss results from any one accident, only the specified total loss for the largest amount will be paid.
Maximum Benefits Available
An employee may participate in the plan under the selected level provided the Monthly Disability Benefit does not exceed 66 2/3% of the monthly salary received from LISD.
The schedule of benefits has ten levels ranging from $200 to $1,800 per month with a corresponding range of monthly premiums. A detailed schedule is available in the LISD Risk Management Office.
Washington National Short Term Disability Income Plan- Plan 2
The monthly benefit of the level selected is payable during each period of total disability. Benefits start on the 91st full day of disability and continue for a period of time not to exceed 24 months for each disability. An employee may participate in the plan provided the benefits under Plan 1 and 2 do not exceed 66 2/3% of monthly salary from Lubbock Independent School District (see booklet for rates).
Washington National Supplemental Long Term Disability Plan-Plan 3
An employee may participate in the plan under any level provided the benefit level does not exceed the level selected for Plan 1. The monthly disability benefits (adjusted as described in this section and in the section "Important Information") for the Supplemental Long Term Disability Plan are payable starting after the benefits of the Base Plan have expired. This is payable for disability due to sickness only. The adjusted monthly benefit will continue to be payable for total disability for the longest of the following benefit periods:
- to age 65
- until payment under the "Base" Plan and the Supplemental Long Term Disability Plan totals four years, but not beyond age 70; or at least
- one year under the Supplemental Long Term Disability Plan.
The monthly benefits of the Supplemental Long Term Disability Plan 3 will be reduced by any amount that the insured is entitled to receive from any Group Disability Insurance Plan, Social Security, including Primary and Dependents' benefits, State Teachers' Retirement, Workers' Compensation or any similar laws, or any other program providing disability or retirement benefits, whether or not such benefits are applied for.
However, the benefits under this plan will not be reduced to less than 25% of the amount of the monthly benefits otherwise payable. Monthly salary will be determined by dividing the educator's regular annual salary by twelve.
Washington National Hospital Indemnity Plan Benefits - Plan 4
The Hospital Indemnity Plan Benefits are as follows:
- $20 - Per day if hospitalized as a result of a covered accident or sickness with a maximum of 365 days for any one period of confinement
- $40 - Per day if in an Intensive Care Unit due to accident or sickness with a maximum of 30 days for any one period of confinement
- $20 - Per day while hospitalized for cancer, heart attack, or stroke with a maximum of 730 days of confinement

Union Mutual Long Term Disability Insurance
What is long term disability insurance?
Long Term Disability Insurance is quite a technical descriptive title, but merely means the Plan guarantees that an insured employee will receive an income after the first 90 days of disability.
Who is eligible and when?
Present Employees- All Superintendents, Deputy Superintendents, Assistant Superintendents, Directors, Consultants, Coordinators, Principals, Assistant Principals, Vice Principals, Supervisors, Assistant Directors, Assistant Coordinators, and Assistants to Superintendent, less than age 65 are eligible to participate on the date of their employment on a full-time active basis.
Future Employees- All Superintendents, Deputy Superintendents, Assistant Superintendents, Directors, Managers, Consultants, Coordinators, Principals, Assistant Principals, Vice Principals, Supervisors, Assistant Directors, Assistant Coordinators, and Assistants to Superintendent, less than age 65 are eligible to participate on completion of 30 days full-time active employment.
How does the plan work?
If an employee becomes totally disabled as a result of sickness or accident arising "on or off the job" he becomes eligible to receive benefits under this plan.
Benefits
If you are totally disabled from accident or sickness, you will be paid 60% of basic monthly earnings to a maximum payment of $5,000.
Benefits Reducedby Other Income Sources
These benefits will be reduced by the following income benefits from other sources:
- any amount payable under a Workers' Compensation or Occupational Disease Act or Law
- benefits payable under any State Disability Benefits law or act
- benefits payable under any other group Disability Income Plan
- the amount of any disability income benefits the insured employee received from any pension program purchased through his employer
- primary and family benefits under the Social Security Act
How long are benefits payable?
For as long as the employee is totally disabled as a result of...
- sickness, to age 65
- accident, to age 65
Termination of Benefits
Insurance terminates at age 65 or when employment terminates, unless a disability has already commenced and the individual is not yet age 65. If you are receiving benefits when your Union Mutual employment terminates, you will continue to receive benefits as long as you are disabled according to the terms of the contract.
