A Note from the School Nurse
I would like to welcome you to
Smylie Wilson. Listed below you will find health policies that will
ensure a healthy start for your child.
Health
History
In order to take the best possible care of your child, please notify
the nurse of any health concerns that could affect your student
during the course of the school day.
Immunizations
Students in middle school will need a booster of certain
immunizations. The nurse will notify you when your child has an
immunization need. An authorized copy of immunizations must be
provided in order for your child to remain in school. A physician
or the Health Department must sign the immunization record.
Illness
Please call the school attendance number, 766-0799, when your child
is ill. Please give the name of the student, grade level, and type
of illness.
Students with a temperature of 100
degrees or above should not attend school. The
student should be fever free
for 24 hours before returning to school. A student who is vomiting
should stay home until he/ she is able to go 24 hours without
vomiting
Communicable Disease
Diseases such as Strep Throat and Scarlet Fever must have a
physicians statement indicating the student is no longer contagious
and may return to school.
Medication
LISD employees are authorized by state law to give medicine at
school only when it is sent in the original container and
proper forms are completed by guardian indicating the date, time,
and amount to be given. Please do not send medication in a Ziploc
with a note to the teacher or myself. I cannot give medication sent
this way to your child. A doctor’s note is mandatory for all
students that carry an inhaler or Epi pen. Please see the nurse for
the forms needed.
Emergency
Number
Please list the name of someone who may be called in case of
emergency. This is very important in helping us locate you or
someone who can help your child. If you have a change in the number
you list please contact the office to update this information.
