Technology Resource Request Forms

The LISD Technology Plan bases the distribution of technology resources on the initiative of teachers and campuses. As you qualify for each incentive, it is your responsibility to request the incentive by filling out a request form listed below. Since the plan’s inception there has been a two year time line for completing the program. Only teachers in their initial two years with LISD can qualify for level II incentives. All teachers who complete level III are eligible to apply for competitive grants under Mini-grants/pilot projects.

The goal of the technology competencies program is to provide teachers with the resources and skills needed to effectively integrate technology into their curriculum. Specific incentives are available at various points in the process to enable all teachers to earn the pieces needed to form a complete Teacher Toolkit. The required competencies are composed of a total of fifteen individual competencies arranged into three levels. The full Toolkit is comprised of a computer, standard software (Windows, MS Office, McAfee Virus Shield), network and Internet connection, a printer, and a computer projection device.

Teachers who have been in the district for 2 years or more cannot qualify for incentives until level III.

Completing Level I qualifies teachers for a computer with MS Office and McAfee Virus Shield. This computer is not intended to add more computers to a classroom, but rather to install the first computer in the classroom. Since most classrooms are already equiped with a computer there is no need to complete a request form for this level.

 

Teachers who complete any two assessments in Level II qualify to receive a printer, if needed. Complete this on-line form, if you need a printer for your classroom and when you have finished two of the Level II competencies.

Level II Part 1 Online Incentive Request Form

Completing the other five assessments in Level II qualifies you for a projection device to display your computer screen to the whole class. Complete this on-line form when you have finished the remaining five Level II competencies.

Level II Part 2 Online Incentive Request Form

Application Deadline: Nov. 13th & Apr 13th

The purpose of this program is to improve instruction by encouraging and supporting teachers' efforts to develop innovative and effective instructional practices. This program provides technology resources beyond those received through the LISD Technology Competencies Level I-III. Special consideration will be given to projects that include at least some of the following areas: information fluency skills, problem-solving skills, authentic audiences, academic integration, and collaboration with other teachers, parents, and the community.
