Senior End of Year Requirements for Graduation

Seniors are required to complete a series of required jobs prior to officially checking out and being cleared for graduation. Start taking care of jobs early so that you'll have everything completed before senior checkout.
LINKS WILL BE LIVE UNTIL FRIDAY, MAY 19th. JOBS NOT COMPLETED BY MAY 19th WILL NEED TO BE COMPLETED AT SENIOR CHECKOUT. THIS WILL SLOW YOU DOWN SIGNIFICANTLY.
COMPLETE YOUR END OF YEAR SURVEY
All seniors must complete the end of the year survey. This information is used to track plans after high school. Click on the senior survey link below to complete this requirement. Visit with your counselor should you have questions.
SIGN UP TO HAVE YOUR FINAL TRANSCRIPT SENT TO YOUR DESTINATION
A final transcript must be sent to either your home or your college / university or technical school. Click on the link below and complete a final transcript request. Your first transcript request is free. Any additional transcripts are $1. See Ms. Floyd, Registrar in the main office if you have questions.

TURN IN ALL LIBRARY BOOKS
Please make sure that you have turned in any borrowed books and paid any accumulated library fines. See Mrs. Arterburn in the Library with questions.
TURN IN ALL TEXTBOOKS
Textbooks will be due no later than Friday, May 19th. Ms. Castillo in the E-I Office has a list of any students who have textbooks checked out by teachers. See Ms. Castillo with questions.
TURN IN ALL EQUIPMENT, UNIFORMS, ETC.
Seniors should check with coaches, athletic trainers and fine arts directors to make sure all equipment and uniforms are turned in. Ms. Castillo has a list (in the E-I office) of students that have outstanding fines or equipment. This must be taken care of in order to clear senior checkout.
MAKE SURE YOU HAVE COMPLETED FAFSA OR TURNED IN A SIGNED OPT OUT FORM
It is a Texas state requirement that students either complete the FAFSA (or TAFSA) or sign an opt out form. Find that information here.
TAKE CARE OF SHOT RECORDS IF NEEDED
Visit with the MHS nurses if you have questions regarding your immunization records.
Complete the immtrac form and return to the School Nurses at MHS by May 5, 2023 if you want your immunization record to continue to be tracked in the state database. This helps doctor's ease of record retrieval.
