New Hire Requirements:

1. Transcripts
- From your most recent enrollment, either high school or college
- Can be an unofficial copy
- Should be uploaded as an attachment with your application

2. Form I-9 Employment Eligibility
- Hard copy of this document is required. Click here to download.
- Complete page 1 only.
- Must be accompanied by acceptable documents (see page 3).

3. Identification
- Valid driver's license or state-issued identification card
- Social Security Card - last name must be the same as on your application
- Both IDs must be originals, copies will NOT be accepted.

4. TEA authorized fingerprint record
- This is required for all school district employees.
- Fingerprints recorded for other entities are not transferrable.
- Scheduling details will be provided by Lubbock ISD if required.
- Applicants/employees are responsible for the fee associated with fingerprinting.

5. Substitute Paperwork

6. Oath of Office
- Hard copy of this document is required. Click here to download.
- This document must be signed with a notary present and notarized.
- Lubbock ISD will notarize this document at no cost if presented in the Human Resources office.

7. Payroll Documents
- Form W-4 Tax Withholding (paper or online)
- Direct Deposit Authorization (paper or online)
- If submitting online, you will do this post-hire in the employee service center. Instructions provided upon hire.
