Texas State Law requires all students to have a complete Immunization Record with in 30 days of enrollment.
Medication at School
In order for students to have access to medication during the school year, parents must follow the following guidelines.
- All medications (Over the Counter & Prescription) must be in the original container, not expired, and no more than a 30 day supply will be kept in the Clinic.
- NO medication (except rescue inhaler, epipens, and Diabetic Supplies with MD orders) will be carried by a student.
- Each medication must have a completed LISD Medication Administration Request Form
- Medications must be picked up before the end of the school year. Medications are not stored over the summer. All Medication not picked up by parent will be destroyed. Medications can not be sent home with a student.
